MINERAL WELLS ISD RE-ENROLLMENT

  1. Mineral Wells ISD Cafeteria plan year will have a short Plan year this year and will run from February 1, 2012 through August 31. Then each plan year thereafter will begin September 1 and run through August of each year.


  2. NBS will serve as the NEW Third Party Administrator (TPA) for Mineral Wells ISD Flexible Spoending accounts, (i.e., Medical and Child Care Reimbursement), for the coming plan year. All participants in the flex plan will receive new flex debit cards for the Medical Flex Spending accounts. All funds in the current MGM accounts must be accounted for prior to January 31, 2012.


  3. Receipts for ALL Medical expenses should be kept in case of audit. Remember that the debit card cannot be used for dental expenses.


  4. The Direct Reimbursement dental plan is offered again and there will be some changes made. The Low Option dental will no longer be made available. Good News! The High Plan is available for employees to choose from and the premiums will actually be decreased for the coming year. See plan information under the Benefit Highlights link / Dental.


  5. The Vision plan with Humana/CompBenefits will be offered again this year. The rates will remain the same for the coming year. See additional plan information and rates under the Benefit Highlights link/Vision.


  6. Unum continues as the Disability carrier. Employees may still enroll and/or increase their monthly benefit amount up to 66 2/3% of gross salary without evidence of good health. *Guarantee issue amounts are subject to pre-existing condition limitations. There are two plan options to choose from; Option B for a 2-year benefit payout; Plan A benefits continue to pay to age 65.


  7. The term life carrier continues to be 5 Star Life for the coming year. Employees can apply for 7 times annual salary up to $500,000, 50% of employee election up to $250,000 for a spouse and 50% of employee election up to $10,000 for children! New hires are guaranteed acceptance on the plan which means no health application is required.


  8. American Public Life will continue as the offering for the Cancer insurance for the coming year. Previous grandfathered cancer plans will continue to be payroll deducted but the carriers are no longer accepting new applications. The APL plan is a group plan which means all applicants are guaranteed acceptance on the plan subject to pre-existing condition limitations.


  9. Continued benefit plan options include: Direct Reimbursement Dental, Vision, Disability, Cancer, GroupTerm Life insurance, Permanent Term life, and Flexible Spending Accounts.
